Home Options Path to Empowerment (H.O.P.E.) helps you move into your own home in the community and supports you so you are safe, comfortable, and happy.
How we help you
As you can see in the middle of our logo, there is a heart – it represents YOU. We listen to you carefully and develop a plan around your needs, hopes, and goals, making sure you are in the center of everything we do.
You will have your own Home Team of caring and skilled professionals who will help you find the home that’s best for you, support you every step of the way, and make sure you are ready for your move.
We also partner with these experts in aging, mental health, and housing: Aging Care Connections, Heartland Human Care Services, North Shore Senior Center, and PLOWS Council on Aging. You will be in good hands!
To participate in H.O.P.E., you need to:
- Live in a nursing facility in Cook County
- Be at least 18 years old
- Qualify for Medicaid
How the H.O.P.E. program works
Step 1:
Our reliable Home Team of licensed social workers and nurses is at your facility every day so you can ask us questions and share concerns. We will be wearing clothes with our HOPE logo – please come up and talk to us any time!
Step 2:
We talk with you and your significant others and/or guardian to find out your hopes and concerns about moving.
Step 3:
TOGETHER, we create a plan to help you move.
Step 4:
We make sure you are ready to move and that it will be safe and comfortable for you.
This includes helping you:
- Find the right home
- Set up your home and make it accessible for you
- Shop for furniture, appliances, and groceries
- Set up services you need such as food delivery, medication delivery, or house cleaning
- Get your benefits such as SNAP, SSI, and Medicaid
- Connect you to support in your community
- Manage your health and finances
We will also introduce you to other people who have gone through the same program so you can talk to them about their experience. They will be able to answer your questions, show you how to use public transportation, and go to community programs with you.
Where will I live?
You are offered a choice about the home and community you would like to live in.
Your options include:
- An apartment or home on your own
- An apartment or home with a family member
- A community group home
- Supportive living facility
How will I be supported in my new home?
We support you every step of the way, even after you move.
We will support you with home and phone visits, and we have money to help support you financially as you get comfortable in your new home.
We will help you learn how to:
- Set a budget and pay bills
- Grocery shop
- Cook
- Do laundry
- Clean your home
- Find your way around the community
You will have a care manager who is your person to call with any questions or concerns. This person will work with you for at least 18 months to make sure you are doing well!

You’ll be able to talk to us at your facility –H.O.P.E. by AgeOptions will be serving patients through telehealth during the national public health emergency. Telehealth — sometimes referred to as telemedicine — is the use of 2-way video conferencing that allows us to have safe and confidential conversations with you. It’s easy to get started!
