This program can help you with everything you need to successfully move from a nursing home back into the community. We promote well-being, support quality of life and strive to bring hope by respecting your unique circumstances and goals.
helps individuals, 18 years and older, who have been living in a nursing home in Cook County for 60 days or more, and who are eligible for and/or on Medicaid, transition back to the community. We assist with skill building, services, supports, housing location and financing, and ongoing case management to ensure a safe and successful transition from nursing home to community.
…you are eligible because you are a class member and part of the Colbert Consent Decree.
The Colbert Consent Decree is the result of a lawsuit against the State of Illinois, because Mr. Colbert wanted more options for care other than nursing facilities. The result of the lawsuit is that the State of Illinois agrees to provide support and services to help class members (like you) move to the community, with the necessary support and assistance you need.
There is no cost to you to participate in the H.O.P.E. program!
